Every year approximately 1.7 million Americans sustain a traumatic brain injury (TBI), and it is conservatively estimated that 3.17 million Americans civilians are living with a permanent TBI-related disability. Additionally there are 4.7 million Americans who are affected by stroke each year. These statistics make brain injury the second most prevalent disability in America. Additionally, 22% of American soldiers wounded Iraq and Afghanistan have sustained a traumatic brain injury, making it the signature wound of the Iraq and Afghanistan wars.
Community Transitions is the only brain injury rehabilitation facility of its kind in South Dakota. We are a non-profit agency and provide community-based service at affordable prices with an experienced professional staff. We provide a wide range of supports including: cognitive therapy, counseling (both individual and group), all aspects of community living training including 24-hour care, employment training and placement, as well as individualized case management. Basically, we work with people in all areas of their lives because we know that a brain injury does not just affect a person's thinking. It impacts all areas of their lives.
Community Transitions bridges the gap between traditional rehabilitative therapy and a successful return to community living. Our cutting-edge program addresses the whole person, based upon evaluations and assessments to determine the specific needs of each individual.
