Community Transitions has been providing brain injury rehabilitation services in South Dakota since January of 1988, focusing on cognitive therapy, counseling, employment and independent living.  Community Transitions is the oldest program of its kind in South Dakota and is one of the oldest in the country.  In 2008 we celebrated 20 years of providing quality brain injury rehabilitation services---our friends at Mayo Clinic also celebrated 20 years of quality brain injury service.

In June 2003, Community Transitions signed an agreement with Regional Rehabilitation Institute, enabling our two facilities to work together to provide all aspects of brain injury rehabilitation. 

In November 2003 Community Transitions formed and began facilitating the Brain Injury Support Group of the Black Hills.  The group is open to survivors, family members and professionals.  More information is available about this group in our "Resources & Support Groups" web page. 

In May 2006 we founded and facilitated the first annual Black Hills Brain Injury Conference.  Since then the conference has grown and is recognized as one of the best in the country, having nationally and internationally recognized speakers.  You can find out more about our conference by going to the "News & Events" page of our website.

On February 1, 2008 we moved to a new location to a larger and newly renovated facility.  

In April 2008, Community Transitions began providing cognitive therapy and counseling through telerehabilitation.  We are exploring new ways to bridge gaps in rehabilitation and provide quality services to people with brain injury.

In May 2008, we purchased Bungalow Software which is state-of-the-art speech and language software that works on receptive and expressive language. 

In August 2008, we purchased a driving simulator to further enhance our ability to prepare people to return to a productive lifestyle.

In September 2009 we began providing 24-hour care for people with brain injury, filling another rehabilitation gap in South Dakota.  This piece of our program has enabled people to receive cognitive retraining, counseling, and independent living training (including ADLs) at Community Transitions.

Our Mission, Vision and Value

The mission of Community Transitions is to provide quality brain injury rehabilitation services for individuals with brain injury and assist them in maximizing their abilities. Our business is to develop opportunities for people with brain injury to participate in their communities at the highest possible level.

It is our philosophy that people served by Community Transitions are first and foremost people with abilities. We assert that all individuals are capable of change when provided services appropriate to their needs. We strongly believe that the brain can continue healing and improving throughout a person's life. We believe that each individual has the right to proceed toward increased levels of independence and responsibility within his or her community.

We know that positve family support is an important part of successful brain injury rehabilitaiton.  We encourage positive family support throughout each facet of a person's rehabilitaion with the ultimate goal being successful independence.
